Approximately, 10% of Indians every year fall into poverty because of a hospitalization event. Many more experience considerable financial stress or forego care. While access to basic financial services, say a loan, is helpful to generate some emergency cash, we need more holistic solutions for families to afford healthcare on a sustainable basis.
Dvara Health Finance (DHF) aims to address critical health financing gaps and enable access to high-quality care. Our goal is to improve both financial and health outcomes over time.
We aim to provide innovative solutions that will enable users to exercise more control over their health spending, avoid medical bankruptcy and obtain better outcomes for themselves and their families.
As a part of DHF, the Patient Health Advisor will oversee the ongoing care of patients in relation to outpatient services, strive to achieve health outcome targets, and guide their patients’ healthcare decision-making to the best possible extent.

We are looking for high-performers with high levels of empathy and patient commitment for this role. The position is based out of Bangalore.

Roles and Responsibilities:

Be the first point of contact for patients under your care, within the Dvara primary care network.
Remain available to your patients all week round; over text/Whatsapp/telehealth platforms.
Work hand in hand with doctors, to ensure end-to-end follow up and navigation for the patients under your care. Escalate cases where patients appear to be falling off-track with respect to their mapped medical plan, to the attention of their primary physicians and other clinical staff.
Provide patient-centric support and counselling – and serve as a dependable source of empathy, without judgment.
Provide periodic check-ins on patients (in-person/ tele-) – to ensure adherence/compliance with recommendations made.
Maintain well-documented and frequently updated patient care notes.
Track patient data that is made available to you – and identify concerning behavioral patterns in patients. Work with patients to understand what works best for them, and provide tailored solutions which effect positive lifestyle behavioral change over time.
Be a source of advice on matters pertaining to stress management/mindfulness, nutritional practices, exercise habits and other lifestyle-related facets.
Act as an approachable source of information to avoid a loss of quality of care if patients are too hesitant to re-approach their physician.
Actively engage with patients (and their families) in the community within reasonable limits. They will be expected to commute to interact with their patients, 50-60% of the time. They will be at the primary care facility for the rest of the time.
Be willing to work and continuously learn, as part of a healthcare team.

Desired Candidate Profile:

They will be a strong team player and willing to go the extra mile for patient well-being
The ideal candidate would have atleast 3 years’ experience as medical social workers, nutritionists/dieticians, nurses, behavioral counsellors, or AYUSH practitioners (BAMS or BHMS)
They will be flexible about working hours and be able to respond to patient emergencies
They will be fluent in spoken English and Kannada
They will be proficient in using Word & Excel – and be able to prepare simple reports.
They will have access to personal transportation for home visits in the area

Other Details:

The evaluation process for this position will include:

One open book case assignment, which can be completed offline.
One interview, over Zoom. The assignment in (i) must be completed in order to receive an interview invitation.

Compensation will depend on qualifications & experience. Benefits will include travel and mobile allowance, life, accident and health insurance.
